Psychedelic Light Control Bowman Leisure Industries Model PSA-3L
3 Channel Powered on/untested beyond that, Sometimes the Center light is more well lit while others may be dim, and then I plugged in later and the 3rd light was bright, and then none so its either not working right or needs the music etc. The knobs are original! and near mint.  I haven't found another so clean looking! Selling completely untested and completely ASIS untested:
